A W-9 form is required by the IRS for our company to issue payouts.
To receive your capital credits payout, we must have a completed W-9 on file.
*If we do not have a W-9 from you, you will NOT receive a check from NNTC.
If you are going to open a JOINT membership with the co-op, both parties must fill out a W-9. After you submit the first W9, you’ll refresh the page to fill out a second form.
